Admission Coordinator. I reviewed all referrals, completed all admissions paperwork with resident and family members. I also did all marketing for the facility and had relationships established with area case managers and local hospitals.

As Social Services Director I completed all Social Documentation for my residents, I met with families and conducted Care Plan Meetings as well as Family Council Meetings. I also assisted with customized activities for my residents and with helping my residents adjust to facility placement. I made appropriate appointments as needed for residents. I’ve had many hours of extended education in all areas of Nursing Facility needs of my residents.
