- Perform utilization reviews for all patients being admitted to ensure appropriate level of care on admission.
- Identify patients at risk for readmission and assess for further needs regarding their medical diagnosis, social support and available resources.
- Create individualized discharge plans and educate identified needs for patients and families.
- Coordinate any community resources and equipment, schedule follow up appointments and specialist referrals as identified.
- Ensure compliance with required medicare forms such as MOON and IM forms prior to discharge.
- Coordinate with physicians and care team regarding optimization of bed utilization, length of stay and appropriate level of care during hospital admission.
